My Home Medical
A medical service that provides in person consultation to obtain a Utah medical card in the convenience of your own home or office. This service is for anyone with a qualifying condition who would prefer to meet with a QMP (Qualified Medical Provider) in the comfort of their home or office rather than going in to a clinic. It is meant to be a convenient option for:

Our Providers
Ryan Larsen is a Family Nurse Practitioner board certified in palliative medicine and hospice. He attended nursing school at Weber State University and Simmons University in Boston for graduate school. He enjoys spending time in the outdoors with family.
Dr Bentley McEntire is a medical doctor with 20+ years of experience in internal medicine, palliative care and hospice.
Qualifying Conditions for medical cards in Utah
Persistent pain that has lasted longer than 14 days that has not been controlled with medications
Persistent nausea
Post-traumatic stress disorder (PTSD) that is being treated and monitored by a licensed health therapist
Persistent muscle spasms
Crohn’s disease or ulcerative colitis
Cancer
Epilepsy or debilitating seizures
Alzheimer’s disease
Amyotrophic lateral sclerosis
HIV or acquired immune deficiency syndrome
Cachexia
Multiple sclerosis
Autism
A terminal illness when the patient’s life expectancy is less than six months
A condition resulting in the individual receiving hospice care
A rare condition or disease that is not adequately managed despite treatment attempts using conventional medications
Note: Patients with medical conditions not listed in the law may petition the Compassionate Use Board for a medical card, which will review complete petitions and recommend eligibility on a case-by-case basis
